The Safety Grade utilizes up to 22 national patient safety measures from the Centers for Medicare & Medicaid Services (CMS) and the Leapfrog Hospital Survey, and information from other supplemental data sources. This survey showed that Mary Washington Hospital has the lowest grade in patient safety in the entire state of Virginia.
